Forms Parent's enrolment form - PDF WORD This form should be used if you are a parent or carer enrolling your own children.
School's enrolment form - PDF WORD This form should be used if you are enrolling a child on behalf of a school. A school is defined as any organisation, other than the child's own parents/carers, educating children.
Payment declaration form - PDF WORD This form should be used whenever somebody other than a parent/carer is paying for the student's fees. It must always be used alongside the school enrolment form.
